Conwy West cluster comprises of 12 General Practices, one of which is managed by the Health Board, with a total practice population of 63,461. This cluster is diverse having 3 GP practices located in a rural area covering wide geographical area.

BCUHB Cluster Population Health Directory
The North Wales Population Health Directory (NWPHD) focuses on the public health priorities and provides information on population health need at different geographical levels (including at Conwy West Cluster level) to NHS staff for planning purposes. The directory is on the Public Health Wales website.

Each Cluster is allocated funding to support cluster priorities in line with Welsh Government priorities to improve access and equity of access or to improve the skill mix within primary care, not least to promote the prudent use of resources.

What are we working on?
The cluster is exploring the benefits of a ‘sub cluster’ for the more rural practices within Conwy West who have different priorities to the more coastal practices. This approach will look to improve the development of the maturity of the cluster and better serve all patients across the area
What we have already done
Conwy West Cluster Network Action Plan 2016-17
The Community Navigator service has integrated itself into the cluster and feedback is extremely positive with good patient outcomes and successful navigation. This service has contributed to the positive relationship between third sector and Primary Care. The evaluation of this service is ongoing.
Diabetes Nurse
The cluster identified the need to improve diabetic care for patients in primary care. The nurse is currently full time and started working with the cluster in May 2016. The nurse up skills primary care clinical staff in the best care for diabetic patients and also educates staff to reduce the need for referral into secondary care.
Mental Health Counselling
The implementation of Mental Health counselling in Conwy West has been similar to Conwy East. The same outcomes and benefits are being collected and the waiting time reduction has also been achieved.
